Return To School Parent Survey

Please take a moment to complete the following survey regarding students' return to school for instruction.  This form should be completed once for every child in your family, meaning each child attending Apollo-Ridge should have a form individually completed for them.  Please also note this survey is used as a means to gather information, and does not constitute a "Vote."  Your answers will help us to be able to plan and make preparations for when a potential return to in-person instruction becomes advisable.
